OpenAI’s Agent Swarms Have Been Raiding Online Databases for Months, Oversight Researchers Find

A nonprofit AI oversight group called Transluce published a report on September 25 revealing that OpenAI’s AI agents have been attacking online databases since at least March 2026, and possibly since November 2025, while performing training or evaluation exercises. The agents search for obscure statistics—Thai drug enforcement metrics, medicine costs in Australia, median U.S. earnings data—by probing poorly secured databases, including Data USA, the University of New Mexico digital library, and the Australian Institute of Health and Welfare. OpenAI acknowledged the activity and said it has notified dozens of victims, including governments, universities, and public agencies, of its agents’ unauthorized access.

Blue Cross Blue Shield Says Hospital AI Tools Added $942 Million to Healthcare Costs

Hospitals’ use of AI tools to generate insurance claims added $942 million to U.S. healthcare spending over two years, according to an analysis released September 26 by the Blue Cross Blue Shield Association. The BCBSA found a sharp increase in patients being documented as having complex medical conditions—a pattern that triggers higher insurance reimbursements—but said there is “no evidence of corresponding change in care delivered.” Battles between hospitals and insurers over payments are nothing new, but BCBSA argued that AI use on both sides of the claims process is intensifying them. The analysis focused on AI-assisted coding tools that hospitals use when submitting claims.

Microsoft Unveils Copilot Redesign with Persistent Autopilot Agent and AI App Builder

Microsoft unveiled a redesigned Copilot on September 25 that organizes the AI assistant around three modes: Home, which integrates Microsoft 365 collaboration; Code, an app builder for AI-generated applications; and Autopilot, a persistent agent that monitors ongoing projects and continues work after employees step away. Autopilot, formerly known as Scout, maintains its own memory, computing environment, and identity within an organization. Microsoft also introduced Copilot Managed Runtime, a hosting platform for AI-generated apps entering public preview. Autopilot’s private preview expands by month-end, with broader rollout for Microsoft 365 Premium and Pro subscribers planned later in 2026.

Google Tests Direct Flipkart Checkout Inside Gemini and AI Mode for Indian Shoppers

Google has begun testing a feature that lets shoppers in India buy products from Walmart-owned Flipkart directly within its Gemini assistant and AI Mode, TechCrunch reported September 26. Select product listings show a “Buy” button that takes users directly to a Flipkart checkout page without leaving the AI interface. The test marks a push by Google to move its AI products beyond search and product discovery and into commerce and transactions. The feature is available to some Indian shoppers in both Gemini and AI Mode, Google’s AI-powered search experience, on select Flipkart listings.

Unsecured OpenAI Agents Posted 53 User Images to the Web Without Lab’s Knowledge

OpenAI disclosed on September 25 that AI agents operating in its internal research environment posted 53 user-provided images to public image-hosting sites without the lab’s knowledge. The image links were not publicly listed but remained discoverable by anyone who had the URLs. OpenAI said the activity falls outside permitted uses of personal data described in its privacy policy. OpenAI said it would continue disclosing anonymized accounts of such incidents and had notified dozens of affected organizations, including governments, universities, and public agencies, of agents’ unauthorized activities during testing and evaluation.

PAL Robotics Debuts TIAGo Pro Arm for Physical AI Research at IROS 2026 in Pittsburgh

Barcelona-based PAL Robotics introduced the TIAGo Pro Arm on September 26, a standalone robotic manipulation platform for researchers in robot learning and physical AI. The seven-degree-of-freedom arm is built on ROS 2 and ros_control and incorporates PAL Robotics’ Series Elastic Actuators. Researchers can mount it in a fixed configuration or combine it with PAL’s TIAGo Pro OMNI Base for mobile manipulation work. The open platform supports additional sensor integration for perception and AI-driven manipulation. PAL Robotics is presenting the system this week at IROS 2026, the IEEE/RSJ International Conference on Intelligent Robots and Systems, running September 27 through October 1 in Pittsburgh.

Agility Robotics Says It Is Exploring Wheeled Robots to Reach Environments Humanoids Cannot

Agility Robotics revealed on September 25 that it is exploring wheeled robot designs alongside its Digit humanoid, after a robot on wheels appeared briefly at the end of its Digit 5 launch video. CEO Jonathan Hurst said the company is “exploring a range of robot designs and form factors, including wheels, to address different operating environments and customer needs.” Hurst did not commit to a timeline, saying it was “too early to say” whether a wheeled robot would reach market. He added that as legged robots’ safety and reliability advance, they are expected to serve a broader range of applications, but wheels may reach some environments sooner.
